SCSI Events (June 2000 +)

The following list identifies all the SCSI-related events that may occur on any hardware monitor. The events are generated by the Default SCSI Decode/Monitor Library.

These events are in addition to the events specifically generated by each individual monitor.

Use this message listing for versions of Hardware Monitors from June 2000 and forwards (IPR 0006 and later).

The event descriptions are in numerical order by event number. (To quickly find an event number, use the "Find" function in your browser.) If you don't find the desired event in this document, try looking in SCSI Events (Feb 99 - March 00).

The string "0xXX" in the event listings means "ANY VALUE". For example, if an event is listed with SCSI Status = 0xXX, then the event description applies for any value of SCSI Status.

At the end of this document is a table of events, ordered by Sense Code, Qualifier, Key, SCSI Status and Device Class.

Some events may have more than one entry, for different combinations of "Device" and the three elements of SCSI sense data:

• Additional Sense Code ("Code", "C"),
• Additional Sense Code Qualifier ("Qualifier", "Q").

In most cases, the default SCSI library expects to get the information from a particular device class. For these cases, the Dev field will list the device class. Otherwise, if the library does not depend on which device is sending the information or if the combination of Code/Qual/Key is valid for all types of devices, the Dev field will be listed as ALL.

Event 100027

• Problem Description: The device has received an I/O request which it cannot process because it is in the process of configuring itself. The request will be re-tried.
• Probable Cause / Recommended Action: If diagnostics are being run on this device, that may be the source of this problem. Wait until the diagnostics are complete before attempting to use the device for normal I/O.
  Alternatively, the device could be in a state from which it cannot extract itself. Cycle power to the device.
  Alternatively, the device has experienced a hardware failure and should be repaired or replaced, as necessary.

Event 100028

• Event Summary: Device cannot be used
• Problem Description: The device was not ready to process requests when it received a request from the device driver because its media is being formatted.
• Probable Cause / Recommended Action: A process has attempted to access the device while another process is formatting its media. Wait until the format is complete before attempting to access the device.

Event 100029

• Event Summary: Excessive errors for device
• Problem Description: The device has met a pre-set threshold condition for its internal error log.
• Probable Cause / Recommended Action: Examine the error logs to determine which parameter met the condition. If the condition being tested involves excessive media errors, the medium may need to be replaced. If the medium is non-removable, repair or replace the device, as necessary.

Event 100034

• Problem Description: The device reported that it was unable to perform the requested select or reselect operation. Data loss may occur as a result.
• Probable Cause / Recommended Action: There could be more than one device having the same address on the SCSI bus. Make all the addresses on the SCSI bus unique, removing devices to reduce the number to seven or less.
  Alternatively, the device has experienced a hardware failure. Repair or replace the device, as necessary.

Event 100035

• Code:0x07 Qualifier:0x00 Key:0x04 Status:0xXX

• Event Summary: Hardware configuration error
• Problem Description: While attempting to process an I/O request, the device driver was unable to proceed because multiple peripheral devices were in a selected state. The request was not processed.
• Probable Cause / Recommended Action: There could be more than one device having the same address on the SCSI bus. Make all the addresses on the SCSI bus unique, removing devices to reduce the number to seven or less.
  Alternatively, one device on the SCSI bus has failed to respond to a deselection request. Isolate and repair or replace the failing device, as necessary.

Event 100038

• Event Summary: Media failure
• Problem Description: The format of the medium in the device is corrupt. The medium is unusable.
• Probable Cause / Recommended Action: A format operation in progress on the device may have been interrupted. Restart the formatting process.
  Alternatively, the medium in the device is flawed. If the medium is removable, replace the medium.
  Alternatively, if the medium is not removable, the device has experienced a hardware failure. Repaired or replace the device, as necessary.

Event 100039

• Problem Description: The device was unsuccessful in performing a format operation upon its medium. The medium is unusable.
• Probable Cause / Recommended Action: Reformatting the medium may fix the problem.
  Alternatively, the medium in the device is flawed. If the medium is removable, replace the medium with a fresh one.
  Alternatively, if the medium is not removable, the device has experienced a hardware failure. Repair or replace the device, as necessary.

Event 100044

Event 100054

• Event Summary: Hardware failure
• Problem Description: While attempting to process an I/O request, the device driver received a status from the device indicating that it could not perform the requested copy data operation because the device adapter could not disconnect from the SCSI bus. The I/O request was not processed.
• Probable Cause / Recommended Action: The device adapter is in a state where it could not logically disconnect from the SCSI bus. Perform a system shutdown. Cycle power to the computer and wait for it to reboot. If this does not correct the condition, the device adapter has experienced a hardware failure. Replace it.

Event 100056

• Problem Description: The device has failed its power-on diagnostics for a specified field-replaceable unit.
• Probable Cause / Recommended Action: The device has experienced a hardware failure. Repair or replace the device, as necessary.
• Details: The combination of Additional Sense Code and Sense Qualifier (0x4000) indicates: Power-on or self-test failure for FRU indicated by sense code qualifier.

  NOTE: valid values for the Additional Sense Code and Sense Qualifier for event #100068 range from 0x4000 to 0x400FF, where 0x40 is the Additional Sense Code.

Event 100074

• Code:0x05 Qualifier:0x00 Key:0x04 Status:0xXX

• Event Summary: Device connectivity or hardware failure
• Problem Description: The device was unable to select one of its logical units.
• Probable Cause / Recommended Action: If the device is a disk array, one of its drives was pulled out physically. This in itself will not cause data loss because the disk array can rebuild the data.
  Alternatively, if the device is not a disk array, check the following. The device may have been powered off and being powered on.
  Alternatively, one or both of the terminators on the SCSI bus may be missing. Put the terminators back in their proper locations at the ends of the SCSI bus.
  Alternatively, the SCSI cable may have become detached from the device. Re-attach the cable.
  Alternatively, the SCSI cable may have failed. Replace it.
  Alternatively, the device may be in a state where it could not process this, or any, request. Cycle power to the device.
  Alternatively, there could be more than one device having the same address on the SCSI bus. Make all the addresses on the SCSI bus unique
  Alternatively, the total length of all cable segments on the SCSI bus exceeds 25 meters. Replace one or more cable segments until the total length is less than this value.
  Alternatively, if all of the above fail to correct the problem, the device has experienced a hardware failure. Repair or replace the device, as necessary.

Event 100075

• Severity: CRITICAL
• Event Summary: Device connectivity or hardware failure
• Problem Description: The device was unsuccessful in its attempt to communicate with one of its logical units due to a parity error. The corresponding I/O request may have been processed in a way which could cause damage to or loss of data.
• Probable Cause / Recommended Action: A segment of the SCSI bus within the device may be damaged. Replace the damaged segment.
